First Amendment to Agreement with Stars Behavioral Health Group for the Time Period of July 1, 2014 through August 31, 2016
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the first amendment to the September 16, 2014 agreement for services with Stars Behavioral Health Group for Transition to Independence Process (TIP) Model technical assistance and training services for the period of July 1,2014 through August 31, 2016; and direct the Clerk of the Board to return two (2) signed originals of the agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Children and Family Services Administration.
