Agreement with Humboldt Family Service Center for the Provision of Mental Health Services for Fiscal Years 2016-17 and 2017-18
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the agreement with Humboldt Family Service Center (HFCS) for the provision of mental health services to Medi-Cal beneficiaries (Attachment 1) for fiscal years 2016-17 and 2017-18; and direct the Clerk of the Board to return two (2) signed originals of the agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Children and Family Services Administration.
