First Amendment to Agreement between Changing Tides Family Services and Humboldt County for the Provision of Mental Health Services for Fiscal Years 2014-15 and 2015-16
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the first amendment to the July 1, 2014 agreement with Changing Tides Family Services for the provision of mental health services (Attachment 1) for fiscal years 2014-15 and 2015-16; and direct the Clerk of the Board to return two (2) executed originals of the of the first amendment to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Children and Family Services Administration.
