RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Authorize the Public Health Director, or designee thereof, to execute fiscal year (FY) 2024-25 Agreement Funding Application (AFA) in the estimated maximum amount of $337,129, Agency Information Form, and Attestation of Compliance with the Requirements for Enhanced Title XIX Federal Financial Participation Rate Reimbursement for Skilled Professional Medical Personnel and their Direct Clerical Support Staff;
2. Authorize the Public Health Director, or designee thereof, to execute all subsequent amendments, and any other agreements, allocations, certifications, attestations, and documents directly related to the attached AFA, including subcontracts, after review of County Counsel, Risk Management, and the County Administrative Office; and
3. Direct the Clerk of the Board to return one (1) copy of the certified Board report to the DHHS - Contract Unit.

DISCUSSION:
Authorized under the Title V of the 1935 Federal Social Security Act, Title V was converted to a Block Grant Program in 1981. Local Health Jurisdictions can request any amount of Title XIX Funding with the understanding that the agency must have the State General Funds and/or additional agency funds to match Title XIX and that spending plans reflect the agency's ability to spend all their Title XIX request.
