DISCUSSION:
On February 7, 2023, DHHS Public Health presented the 2022-2027 Humboldt County Community Health Improvement Plan (CHIP) to the Board of Supervisors. At that meeting, DHHS Public Health announced that work was underway to develop the Live Well Humboldt Community Health Dashboard to share community health data more broadly with partners and community members. The Board has supported the work to develop this dashboard through the approval to announce a Request for Proposal (RFP) for a vendor to help build and maintain a community health dashboard. DHHS Public Health received five proposals in response to the RFP and through a process of collaborative review chose Conduent Healthy Communities Corporation.

Live Well Humboldt is a network of community collaborators committed to aligning community efforts across sectors and focus areas to advance health and racial equity and improve health for everyone in Humboldt County.

The Live Well Humboldt Community Health Dashboard will be available to the public beginning the last week of November 2023 via livewellhumboldt.org. The dashboard is a free online resource to access community quality of life data and related resources, like funding opportunities, promising practices, and reports.
