First Amendment to State Standard Agreement #15-10156 with California Department of Public Health for the California Home Visiting Program
Recommendation
That the Board of Supervisors approve the first amendment (AOl) to State Standard Agreement #15-10156 for the term of July 1, 2015 to June 30, 2019; authorize the Chair of the Board to sign three (3) originals of the first amendment; direct the Clerk of the Board to return two (2) executed originals of the first amendment and one (1) fully executed Board Agenda Item to the Department of Health and Human Services (DHHS), Contract Unit for forwarding to DHHS-Public Health; and authorize the Public Health Director or Designee to sign any subsequent amendments and documents directly related to the State Standard Agreement #15-10156.
