Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the Application Service Provider Agreement between DHHS - Public Health and Persimmony International, Inc.; and direct the Clerk of the Board to return the signed documents and one (1) copy of the Board motion to the Department of Health and Human (DHHS) Services Contract Unit for transmittal to DHHS-Public Health.
