RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Adopt the attached resolution which authorizes the Department of Health and Human Services Director, or a designee thereof, to execute and deliver Program Funding Agreement Number 7480-CACCE-CP_2057_HUMBOLDT-01-G with HORNE as successor for Advocates for Human Potential, Inc. as the third-Party Administrator for the Community Care Expansion Preservation Program, along with any and all other agreements, certifications, attestations and documents, including, without limitation, any and all agreements with other entities, and any and all amendments thereto, that are required or deemed necessary or appropriate to evidence and secure funding made available by the California Department of Social Services through the Community Care Expansion Preservation Program and the County of Humboldt's obligations related thereto;
2. Approve the attached supplemental budget request in the amount of Four Hundred Fifty Thousand Six Hundred Seven Dollars ($450,607.00) for Department of Health and Human Services - Social Services budget unit 1160-511 (4/5 Vote Required); and
3. Direct the Clerk of the Board to return one (1) fully executed certified copy of the attached resolution to the Department of Health and Human Services - Contract Unit for further processing.
