Agreement between Humboldt County Department of Health and Human Services and Remi Vista. Inc. for Transitional Housing Program - Plus - Foster Care (THP-Plus-FC) Services for Fiscal Year 2014-15
Recommendation
That the Board of Supervisors approve the Agreement between County of Humboldt Department of Health and Human Services (DHHS)-Social Services and Remi Vista, Inc. to provide Transitional Housing Program - Plus - Foster Care (THP-Plus-FC) services for eligible Non-Minor Dependent (NMDs) youth (ages 18 up to 21), who choose to continue in Extended Foster Care (EEC) as dependents or wards of the Juvenile Court; authorize the Chair to execute three (3) originals of the Agreement; direct the Clerk of the Board to return two (2) fully executed originals of the agenda item and agreement to DHHS-Contract Unit for forwarding to DHHS-Children and Family Services Administration; and authorize the DHHS-Assistant Director of Programs to exercise, on behalf of DHHS-Children and Family Services, the first and second options to extend the Agreement under the same terms and conditions for additional one year terms from July 1, 2015 through June 30, 2016 and July 1, 2016 through June 30, 2017.
