RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with North Coast Health Improvement and Information Network regarding the development, implementation and management of a Community Information Exchange network for the period of March 9, 2021 through June 30, 2023;
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with North Coast Health Improvement and Information Network, after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Clerk of the Board to return one fully executed original copy of the attached professional services agreement with North Coast Health Improvement and Information Network to the Department of Health and Human Services - Contract Unit for further processing.

On Oct. 18, 2019, the California Department of Health Care Services (DHCS) announced the availability of $20 million in one-time funds to counties not participating in the Whole Person Care Pilot program based on the demonstration of interest and letters of support from local partners within each county.
