RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Fortuna Seventh-Day Adventist Church regarding the provision of monitoring services related to the Humboldt Alcohol Recovery Treatment Program for the period of July 1, 2019 through June 30, 2024;
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Fortuna Seventh-Day Adventist Church, after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Clerk of the Board to return one (1) fully executed original copy of the attached professional services agreement with Fortuna Seventh-Day Adventist Church to the Department of Health and Human Services - Contract Unit for further processing.

The Humboldt County Department of Health and Human Services - Behavioral Health ("DHHS - Behavioral Health) has a mission to provide quality substance use disorder services based upon current best practices. The intent of the services provided by Fortuna Seventh-Day Adventist Church's Humboldt Alcohol Recovery Treatment ("HART") Program is to eliminate the behavior of driving under the influence of drugs and/or alcohol.
