Second Amendment to July 1, 2014 Agreement between Remi Vista, Inc. and Humboldt County for the Provision of Mental Health Services for Fiscal Years 2014-15 and 2015-16
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the Second Amendment to the July 1, 2014 Agreement with Remi Vista, Inc. for the provision of Mental Health Services (Attachment 1) for fiscal year 2015-16; direct the Clerk of the Board to return two (2) executed originals of the of the Second Amendment to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Children and Family Services Administration.
