Second Amendment to the Professional Services Agreement with Eureka Pharmacy, LLC Regarding the Provision of Pharmaceutical Consultation Services in the Amount of $92,088end

RECOMMENDATION(S):Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute the attached second amendment to the professional services agreement with Eureka Pharmacy, LLC dated Nov. 23, 2022 regarding the provision of pharmaceutical consultation services in an amount not to exceed Ninety-Two Thousand Eighty-Eight Dollars ($92,088.00) for the period of July 1, 2022 through June 30, 2028; and
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the professional services agreement with Eureka Pharmacy, LLC dated Nov. 23, 2022, after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Cler of the Board to electronically provide the Department of Health and Human Services - Contract Unit with one (1) fully executed certified copy of the Board Order related to this item.
