Approve Agreement between the Humboldt County Department of Health and Human Services and the Redwood Community Action Center for the Transitional Housing Program-Plus (THP-Plus)
Recommendation
That the Board of Supervisors approve the Agreement between County of Humboldt Department of Health and Human Services (DHHS)-Children and Family Services and Redwood Community Action Agency, Youth Services Bureau, to provide Transitional Housing Program-Plus services for emancipated former foster youth (ages 18 up to 24 years); authorize the Chair to execute three (3) originals of the Agreement; direct the Clerk of the Board to return two (2) fully executed originals of the agenda item and agreement to Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Children and Family Services Administration; and authorize the DHHS Assistant Director of Programs to exercise the first and second options to extend the Agreement for additional one year terms from July 1, 2015 through June 30, 2016 and July 1, 2016 through June 30, 2017 on behalf of DHHS-Children and Family Services.
