RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the attached Partnership HealthPlan of California Grant Agreement in the amount of $100,000 for fiscal years (FY) 2025-26 through 2027-28;
2. Authorize the Department of Health and Human Services (DHHS) - Public Health Branch Director, or a designee thereof, to execute the attached Partnership HealthPlan of California Grant Agreement for FYs 2025-26 through 2027-28; and
3. Authorize the DHHS - Public Health Branch Director, or a designee thereof, to execute any and all subsequent amendments, certifications, attestations and documents directly associated with the attached Partnership HealthPlan of California Grant Agreement after review and approval by County Counsel, Risk Management and the County Administrative Office.

DISCUSSION:
Partnership HealthPlan of California is a non-profit community-based healthcare organization that contracts with the State of California to provide Medi-Cal services to 24 counties in Northern California, under a County Organized Health System Model.

The California Department of Health Care Services (DHCS) has encouraged collaboration between Medi-Cal managed care plans and Local Health Jurisdictions (LHJs) to strengthen Community Health Assessments (CHAs) and Community Health Improvement Plans (CHIPs) as a means to improve population health management and align with the DHCS Quality Strategy.
