RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve Grant Agreement # 19-10417 with California Department of Public Health (CDPH), Office of AIDS for the period of July 1, 2019 through June 30, 2024;
2. Authorize the Chair of the Board to sign four (4) originals of the Grant Agreement # 19-10417, one original Contractor Certification Clause (CCC 04/2017), one original Darfur Contracting Act, and one original Civil Rights Form;
3. Authorize the Director of Public Health, or designee, to sign any subsequent amendments or documents directly related to Grant Agreement # 19-10417 upon County Counsel and Risk Management review and approval; and
4. Direct the Clerk of the Board to return the four (4) signed originals and aforementioned certifications to Department of Health and Human Services (DHHS) - Public Health for further processing.

DISCUSSION:
California Department of Public Health's (CDPH) Office of AIDS has contracted with DHHS - Public Health since the late 1980s to provide HIV/AIDS services in Humboldt County. The goal of the HIV/AIDS Surveillance Program is to establish and enhance active and passive HIV/AIDS case surveillance in health and social service settings. Objectives of this program include improving the timeliness, accuracy and reliability of local HIV/AIDS case data; investigating reported HIV/AIDS cases in order to establish an accurate mode of HIV transmission; and in conjunction with CDPH, Office of AIDS staff, conducting investigations of cases that are of public health importance.
