RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Adopt the attached resolution authorizing the Department of Health and Human Services Director, or a designee thereof, to enter into, execute and deliver any and all applications, agreements, amendments, certifications, attestations and other documents necessary to apply for, and accept, if awarded, up to $45,000,000 in grant funding made available through the California Department of Health Care Services’ Behavioral Health Continuum Infrastructure Program for the construction of a new Psychiatric Health Facility in Humboldt County;

2.                     Approve, and authorize the Director of Public Works to advertise and distribute, the attached request for qualifications regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County;

3.                     Authorize the Director of Public Works to execute a consultant services agreement with the successful consultant regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County for an amount not to exceed $150,000, and any subsequent amendments thereto, that is substantially similar to the sample consultant services agreement contained in the attached request for qualifications, after review and approval by the Department of Health and Human Services Behavioral Health, County Counsel, Risk Management and the County Administrative Office; and

4.                     Direct the Clerk of the Board to return one (1) fully executed certified copy of the attached resolution to the Department of Health and Human Services - Behavioral Health for submission to the California Department of Health Care Services.

DISCUSSION:

Sempervirens, the County of Humboldt’s Psychiatric Health Facility, is in need of a new home.  Constructed in 1906 as a General Hospital and renovated into a Mental Health Facility in 1969, the building requires constant maintenance and expensive repairs.  There have not been significant improvements made to the facility in 45 years.  The structure has exceeded its useful life, is not ideal for use by either staff or patients, and modern mental health facility design is largely incompatible with the current structure.

 

The California Department of Health Care Services established the Behavioral Health Continuum Infrastructure Program to address historic gaps in the behavioral health care continuum and meet the growing demand for services and support across the life span of vulnerable individuals in need.  The Behavioral Health Continuum Infrastructure Program provides a possible source for significant funding to design and construct a new modern Psychiatric Health Facility in Humboldt County that will preserve and potentially expand upon existing mental health services for the community.  The Humboldt County Department of Health and Human Services - Behavioral Health  (“DHHS - Behavioral Health”) has additionally identified a need for Mental Health Rehabilitation beds to care for the needs of clients requiring a high level of care.  It is anticipated that the addition of Mental Health Rehabilitation beds would reduce capacity concerns at Sempervirens and may make the application more competitive.  

 

Grant applications for funding made available through Round 1 of the Behavioral Health Continuum Infrastructure Program are due on Dec. 13, 2024.  In order to meet the application requirements, a design team of architects and engineers will be required to prepare schematic design documents for a potential new facility.  Due to the nature of the anticipated services, a qualifications-based selection process is required to procure these services.  The Humboldt County Department of Public Works (“Public Works”), in consultation with DHHS - Behavioral Health, has prepared the attached request for qualifications for the services necessary to submit a completed application. 

 

The total value of the consultant services agreement that is expected to be finalized at the conclusion of the request for qualifications process is estimated to not exceed $150,000.  Even if the county’s grant application is unsuccessful, the schematic design effort that is expected to result from the request for qualifications will provide insight into facility needs as well as better position the county for future grant funding opportunities.  Additional consultants that provide site investigation and real estate due diligence services will be required to complete the grant application, which may be retained directly outside of the scope of the request for qualifications. 

 

Accordingly, it is recommended that the Board of Supervisors adopt attached resolution authorizing the DHHS Director, or a designee thereof, to enter into, execute and deliver any and all applications, agreements, amendments, certifications, attestations and other documents necessary to apply for, and accept, if awarded, up to $45,000,000 in grant funding made available through the California Department of Health Care Services’ Behavioral Health Continuum Infrastructure Program for the construction of a new Psychiatric Health Facility in Humboldt County.  It is also recommended that the Board of Supervisors approve and authorize the Director of Public Works to advertise and distribute, the attached request for qualifications regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility Project in Humboldt County.  Finally, it is recommended that the Board of Supervisors authorize the Director of Public Works to execute a consultant services agreement with the successful consultant regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County for an amount not to exceed $150,000, and any subsequent amendments thereto, that is substantially similar to the sample consultant services agreement contained in the attached request for qualifications, after review and approval by DHHS - Behavioral Health, County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of needed architecture and engineering services.

The request for qualifications and application processes will enable DHHS - Behavioral Health to apply for further funding that will support the construction of a new Psychiatric Health Facility in Humboldt County.  The figures listed in the table above are based on the assumption that DHHS - Behavioral Health would be awarded Behavioral Health Continuum Infrastructure Program funds over a period of five years. 

 

DHHS - Behavioral Health and Public Works will return to the Board of Supervisors with full budget costs and options for securing the required ten percent (10%) project match prior to submitting the grant application.  Types of eligible match sources include, without limitation, cash, In-Kind Property Equity and Sunk Costs.  Potential options for the source of the match may include Finance Plan funding, the value of the property proposed for construction, and will include all costs associated with the request for qualifications, architectural design and site investigation reports as Sunk Costs.  Additionally, if awarded, DHHS - Behavioral Health would return to the Board of Supervisors with subsequent supplemental budgets as necessary.

 

Expenditure appropriations of intergovernmental transfer funds related to the request for qualifications and application processes have been included in the adopted fiscal year 2024-25 budget for DHHS - Behavioral Health budget unit 1160-511.  Intergovernmental transfer funds are used to enhance and strengthen services offered to Medi-Cal participants.  Improving access to Behavioral Health services is an eligible use of intergovernmental transfer funds.  The total projected cost associated with the request for qualifications and application processes is $232,875, 15% of which will be used to cover Public Works estimated administrative costs. 

 

STAFFING IMPACT:

Due to the time constraints related to applying for Behavioral Health Continuum Infrastructure Program Funds, Public Works has redirected considerable resources to this project in a short period of time, which may impact other ongoing Public Works projects.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board of Supervisors may choose not to adopt the attached resolution authorizing participation in the Behavioral Health Continuum Infrastructure Program or approve the attached request for qualifications regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County.  However, this alternative is not recommended because participation in the Behavioral Health Continuum Infrastructure Program provides a unique opportunity to fund a project of significant need in the community.
