RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Mental Health Management I, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2025, through June 30, 2028; and

2.                     Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Mental Health Management I, Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) fully executed original copy of the attached professional services agreement with Mental Health Management I, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Behavioral Health (“DHHS -Behavioral Health”), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in a locked and secure setting.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. 

 

Mental Health Management I, Inc., doing business as Canyon Manor, operates a Mental Health Rehabilitation Center (“MHRC”) located in Novato, California.  Mental Health Management I, Inc’s MHRC provides mental health treatment services to individuals who require twenty-four (24) hour monitoring in a locked and secure residential setting due to the severity of their mental illnesses. 

DHHS - Behavioral Health clients placed at Mental Health Management I, Inc’s MHRC are monitored by the County of Humboldt’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (“SAFARI”) committee.  The SAFARI committee authorizes services, determines the appropriate level of care and monitors clients’ needs in the community upon discharge.  When an initial placement at Mental Health Management I, Inc’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed weekly, in order to transition the client to community living as soon as may be appropriate.  All mental health treatment services provided at Mental Health Management I, Inc MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

On September 20, 2022 (item D-12), the Humboldt County Board of Supervisors (“Board”) approved the current professional services agreement with Mental Health Management I, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2022, through June 30, 2025.  The current professional services agreement with Mental Health Management I, Inc. was subsequently in order to increase the rates of compensation set forth therein on June 21, 2022 and April 23, 2024.  The attached professional services agreement will allow eligible DHHS - Behavioral Health clients to receive long-term residential mental health treatment services at Mental Health Management I, Inc’s MHRC for the period of July 1, 2025, through June 30, 2028.

 

Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Mental Health Management I, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2025, through June 30, 2028.  It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with Mental Health Management I, Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed residential mental health treatment services.

The attached professional services agreement with Mental Health Management I, Inc. has a maximum amount payable of Three Million Two Hundred One Thousand Nine Hundred Three Dollars ($3,201,903.00) for the period of July 1, 2025, through June 30, 2028.  In no event shall the maximum amount paid under the attached professional services agreement with Mental Health Management I, Inc. exceed One Million Sixty-Seven Thousand Three Hundred One Dollars ($1,067,301.00) per fiscal year for fiscal years 2025-2026 through 2027-2028.  However, if local state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Mental Health Management I, Inc. in its entirety. 

 

Expenditure appropriations related to the attached professional services agreement with Mental Health Management I, Inc. have been included in the proposed fiscal year 2025-2026 budget for DHHS - Behavioral Health Adult Services budget unit 1170-496.  Funding for the attached professional services agreement with Mental Health Management I, Inc. is available through state realignment revenues received by DHHS - Behavioral Health. 

 

STAFFING IMPACT:

Approval of the attached professional services agreement with Mental Health Management I, Inc. will not impact current staffing levels.

 

OTHER AGENCY INVOLVEMENT:

None                     

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board may choose not to approve the attached professional services agreement with Mental Health Management I, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses.  However, this alternative is not recommended since it would require DHHS - Behavioral Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
