RECOMMENDATION(S):Recommendation
That the Board of Supervisors:
1. Waive the requirements of the Humboldt County Nuclear-Free Ordinance with regard to the attached transitional rent provider agreement with Partnership HealthPlan of California; and
2. Approve, and authorize the Chair of the Board to execute, the attached transitional rent provider agreement with Partnership HealthPlan of California regarding the provision of transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County; and
3. Authorize the Department of Health and Human Services - Social Services Director, or a designee thereof, to execute any and all subsequent amendments to, and any other agreements, certifications, attestations and documents directly associated with, the attached transitional rent provider agreement with Partnership HealthPlan of California, after review and approval by County Counsel, Risk Management and the County Administrative Office; and
4. Direct the Clerk of the Board to return one (1) fully executed electronic copy of the attached transitional rent provider agreement with Partnership HealthPlan of California to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:
The California Advancing and Innovating Medi-Cal (“CalAIM”) program is a comprehensive, multi-year initiative launched by the California Department of Health Care Services in January 2022 that is designed to transform Medi-Cal into a more integrated, person-centered and equitable health system. One key aspect of CalAIM is the introduction of Community Supports services, which are cost-effective alternatives to traditional medical services that address social determinants of health within local Medi-Cal managed care plans throughout California. Partnership HealthPlan of California is the local Medi-Cal managed care plan in Humboldt County.
As an agency committed to enhancing the health and well-being of the community, the Humboldt County Department of Health and Human Services - Social Services (“DHHS - Social Services”) seeks to participate in this innovative program to provide transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County in interim and permanent settings. Transitional rent services are designed to assist eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness maintain safe and stable tenancy once housing is secured, such as coordination with landlords to address issues, assistance with the annual housing recertification process and linkage to community resources to prevent eviction.
The attached transitional rent provider agreement with Partnership HealthPlan of California will allow DHHS - Social Services to become a transitional rent provider and seek reimbursement for the provision of transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County. It should be noted that the attached transitional rent provider agreement with Partnership HealthPlan of California comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract development process.
Accordingly, DHHS recommends that the Board approve, and authorize the Chair of the Board to execute, the attached transitional rent provider agreement with Partnership HealthPlan of California regarding the provision of transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County. It is also recommended that the Board authorize the DHHS Director, or a designee thereof, to execute any and all subsequent amendments to, and any other agreements, certifications, attestations and documents directly associated with, the attached transitional rent provider agreement with Partnership HealthPlan of California, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays provision of needed transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County.

The attached transitional rent provider agreement with Partnership HealthPlan of California will allow DHHS - Social Services to be reimbursed up to One Thousand Fifty-One Dollars ($1,051.00) per Medi-Cal recipient receiving transitional rent services. The above-referenced proposed revenue and expenditures for fiscal year 2026-2027 are based on the provision of transitional rent services to twenty (20) eligible Medi-Cal recipients, and the above-referenced proposed revenue and expenditures for fiscal year 2027-2028 are based on the provision of transitional rent services to thirty-five (35) eligible Medi-Cal recipients.
STAFFING IMPACT:
Approval of the attached transitional rent provider agreement with Partnership HealthPlan of California will not impact current staffing levels.

ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board may choose not to approve the attached transitional rent provider agreement with Partnership HealthPlan of California regarding the provision of transitional rent services to eligible Medi-Cal recipients who are experiencing, or at risk of experiencing, homelessness within Humboldt County. However, this alternative is not recommended as it would severely limit DHHS - Social Services’ ability to provide needed transitional rent services within Humboldt County.
