Recommendation
That the Board of Supervisors:
1. Approve Grant Number 90FPSG0034-01-00 with Partners in Care Foundation for the period Aug. 1, 2020 and ending July 31, 2023;
2. Authorize the Director of Public Health or a designee thereof to sign two originals of the agreement and all certifications;
3. Authorize the Director of Public Health or a designee thereof to sign all future amendments and documents directly related to this grant agreement upon review and approval of County Counsel and Risk Management; and
4. Direct the Clerk of the Board to return one copy of the certified board report to the Department of Health and Human Services (DHHS) - Contract Unit.

DISCUSSION:
Unintentional falls are a major cause of loss of independence of elders. In Humboldt County, the cost of older adult falls over a 5-year span from 2009-2014 was $244,615,000. The average cost of an older adult fall is $35,000. The rate of older adults visiting the ER due to a fall is 68% higher than the state average.
The Healthy Communities Division of Public Health was offered this funding by Partners in Care Foundation in order to increase local capacity to conduct evidence-based falls reduction programs aimed at older adults. Through this funding, the Older Adult Falls Prevention program provides targeted education via workshops and other tools to reduce the likelihood an older adult fall.
This funding provides Healthy Communities staff resources to outreach to community partners to improve the number of participants in Older Adult Falls Prevention Education. The focus this grant cycle is for the Falls Prevention Program to educate 239 limited English-speaking older adult residents of Humboldt County per year on falls prevention.
This item is coming to your Board after the agreement effective date due to late delivery by the grantor.
FINANCIAL IMPACT:
Approval of the subaward agreement with Partners in Care Foundation will allow the Department of Health and Human Services - Public Health to be reimbursed up to $13,789 for the period of Aug. 1, 2020 through July 31, 2023. There are sufficient appropriations in Budget Unit 414 - Healthy Communities for Fiscal Year 2020-21. The remaining amounts will be included in the annual budget cycles for FY 2021-22 and FY2022-23. There is no impact to the County General Fund.
