RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Department of Health and Human Services (DHHS) Director, or a designee thereof, to execute, the attached memorandum of understanding with American Hospital Management Corporation regarding the transfer of match funding for the construction of a Behavioral Health Crisis Triage Center in Humboldt County as part of the of the California Behavioral Health Continuum Infrastructure Program (BHCIP); and

2.                     Authorize the DHHS Director or a designee thereof, to execute any and all subsequent amendments to the attached memorandum of understanding, after review and approval by County Counsel, Risk Management, and the County Administrative Office; and

3.                     Authorize the DHHS Director, or a designee thereof, to execute any and all agreements, amendments, certifications, attestations, and other documents necessary to accept the $500,000 in grant funding made available through the County Medical Services Program (CMSP), to be utilized as match funding for the BHCIP, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

4.                     Approve the attached supplemental budget in the amount of $500,000 fiscal year (FY) 2024-25 for Behavioral Health (1700424) (4/5 Vote Required); and

5.                     Approve the attached supplemental budget in the amount of $500,000 FY 2024-24 for Opioid Settlement (1600600) (4/5 Vote Required); and

6.                     Direct the Clerk of the Board to provide the DHHS - Contract Unit with one fully executed, certified copy of the Board Order related to this item.

DISCUSSION:

On Jan. 24, 2023, your Board authorized the DHHS - Behavioral Health to submit a joint application with American Hospital Management Corporation, doing business as Mad River Community Hospital, to the California Department of Health Care Services for grant funding made available through the BHCIP in order to construct a Behavioral Health Crisis Triage Center in Humboldt County.  Your Board authorized the commitment of up to $3,000,000 in “match” funding to support the construction of a Behavioral Health Crisis Triage Center.  When completed, the Behavioral Health Crisis Triage Center will consist of approximately 43 new behavioral health crisis beds, including 12 sobering center cots, 12 crisis stabilization unit beds - half for adults and half for children and youth, 9 mental health crisis residential beds and 10 crisis residential beds for those with dual-diagnosis substance use and mental health disorders.

On March 21, 2023, your Board authorized DHHS - Behavioral Health to submit an application to the CMSP for Healthcare Infrastructure Development Matching Grant funding to support the construction of a Behavioral Health Crisis Triage Center in Humboldt County as part of the of the BHCIP.  On May 26, 2023, the CMSP notified DHHS - Behavioral Health of a $500,000 award, conditional on the award of BHCIP funding and the submission of a memorandum of understanding between the County of Humboldt and Mad River Community Hospital.

 

On June 23, 2023, Mad River Community Hospital was notified of an award of $12,361,918 in BHCIP funding for the construction of a Behavioral Health Crisis Triage Center in Humboldt County.  Your Board subsequently adopted the FY 2023-24 Humboldt County Budget, which allocated $1,500,000 in American Rescue Plan Act funds and $500,000 in Opioid Settlement funding to support the construction of a Behavioral Health Crisis Triage Center in Humboldt County, on June 27, 2023.

 

The supplemental budget request for Opioid Settlement funds received by the County of Humboldt will be utilized as required match for the BHCIP grant.  The remaining funds have been committed by DHHS - Behavioral Health, utilizing $107,675 in Intergovernmental Transfer funds.

 

The attached memorandum of understanding with American Hospital Management Corporation will allow the County of to receive CMSP funding in the amount of $500,000 and transfer a total of $2,607,675 allocated by your Board through the state authorized funding portal to be used as match funding for the construction of the Behavioral Health Crisis Triage Center in Humboldt County as part of the of the BHCIP.

 

While DHHS - Behavioral Health and Mad River Community Hospital submitted a joint application for BHCIP funding, the grant award and performance agreement are solely between the State of California and Mad River Community Hospital.  The County of Humboldt is not a party to the performance agreement and has no direct responsibility thereunder.  However, the local match funding committed by your Board as part of the successful joint BHCIP grant application must be transferred to, and utilized by, Mad River Community Hospital before it may use the BHCIP funding awarded thereto.

Narrative Explanation of Financial Impact:

The total amount of County of Humboldt funding to be used as match for the construction of the Behavioral Health Crisis Triage Center is $2,607,675.  These funds were anticipated in the FY 2024-25 adopted budget with the following two exceptions: the Opioid Settlement funds which are now managed out of Special Revenue Fund (1600) and CMSP Healthcare Infrastructure Development Matching Grant funds.  The attached supplemental budgets will allow DHHS - Behavioral Health to utilize Opioid Settlement funds in the amount of $500,000 from Opioid Settlement (1600600) and CMSP Healthcare Infrastructure Development Matching Grant funds in the amount of $500,000 from Behavioral Health (1700424).

 

STAFFING IMPACT:

The recommended actions will not impact current staffing levels.  DHHS - Behavioral Health will return to the Board with a plan for staffing the facility when the crisis triage center facility is built, which is estimated to be FY 2026-27.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board may choose not to approve the attached memorandum of understanding with American Hospital Management Corporation regarding the transfer of match funding for the construction of a Behavioral Health Crisis Triage Center in Humboldt County as part of the of the BHCIP or the supplemental budgets related thereto.  However, this alternative is not recommended because execution of the attached memorandum of understanding is necessary to allow Mad River Community Hospital to access state grant funding to construct a Behavioral Health Crisis Triage Center in Humboldt County.
