RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the attached County-Based Medi-Cal Administrative Activities Targeted Case Management Agreement by and between the County of Santa Cruz and the County of Humboldt for Fiscal Years 2025 through 2028; and
2. Authorize the Department of Health and Human Services - Public Health Director, or a designee thereof, to execute the attached County-Based Medi-Cal Administrative Activities Targeted Case Management Agreement; and
3. Authorize the Department of Health and Human Services - Public Health Director, or a designee thereof, to execute any and all subsequent amendments, certifications, attestations, and documents associated with the attached County-Based Medi-Cal Administrative Activities Targeted Case Management Agreement after review and approval by County Counsel, Risk Management and the County Administrative Office; and
4. Direct the Clerk of the Board to provide the Department of Health and Human Services - Contract Unit with one certified copy of the Board Order pertaining to this item.

DISCUSSION:
The County-Based Medi-Cal Administrative Activities (CMAA) and Targeted Case Management (TCM) Coordination and Claims Administration program provides administrative, programmatic, and fiscal oversight, and support to CMAA and TCM program participants on a countywide basis. The Department of Health and Human Services - Public Health Branch (DHHS - Public Health) serves as the Local Governmental Agency (LGA) for CMAA and TCM claiming on behalf of Humboldt County.
The LGA draws down Federal Financial Participation revenues for DHHS - Public Health to decrease local costs for eligible services and to assist in maintaining service levels. Services include case management, referrals, outreach, and program planning for Medi-Cal services.
Humboldt County has been participating in CMAA and TCM since Fiscal Year (FY) 1997-98. Since this time the CMAA and TCM programs have brought in funds to support community members. The CMAA and TCM programs provide reimbursement for Medi-Cal outreach, client case management, and administrative costs associated with referring clients to Medi-Cal and Medi-Cal services.
The County of Santa Cruz was elected to the Host County position by the Local Governmental Agency Consortium of CMAA participating counties. The responsibilities of the Host County include collecting and disbursing funds for the CMAA and TCM trust funds, paying Department of Health Care Services for the CMAA/TCM administrative costs, and paying the CMAA/TCM LGA.

Narrative Explanation of Financial Impact:
The attached CMAA and TCM Agreement authorizes DHHS - Public Health to reimburse the County of Santa Cruz a portion of the revenue it received in the previous fiscal year for eligible services provided. Total reimbursement under this agreement is capped at $90,000 across three fiscal years. Funding is entirely sourced from state CMAA and TCM reimbursements.

ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board could decide not to approve the CMAA TCM Agreement. However, DHHS - Public Health does not recommend this alternative as it would decrease services to vulnerable populations.
