RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Authorize the Public Health Director, or designee thereof, to execute fiscal year (FY) 2024-25 Agreement Funding Application (AFA) in the estimated maximum amount of $337,129, Agency Information Form, and Attestation of Compliance with the Requirements for Enhanced Title XIX Federal Financial Participation Rate Reimbursement for Skilled Professional Medical Personnel and their Direct Clerical Support Staff;

2.                     Authorize the Public Health Director, or designee thereof, to execute all subsequent amendments, and any other agreements, allocations, certifications, attestations, and documents directly related to the attached AFA, including subcontracts, after review of County Counsel, Risk Management, and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) copy of the certified Board report to the DHHS - Contract Unit.

DISCUSSION:

Authorized under the Title V of the 1935 Federal Social Security Act, Title V was converted to a Block Grant Program in 1981. Local Health Jurisdictions can request any amount of Title XIX Funding with the understanding that the agency must have the State General Funds and/or additional agency funds to match Title XIX and that spending plans reflect the agency’s ability to spend all their Title XIX request.

 

Beginning in 2001, Public Health’s Maternal, Child and Adolescent Health (MCAH) division has completed the Agreement Funding Application (AFA) and received Title V and Title XIX funds to manage the program. The AFA provides a Scope of Work (SOW) guided by several public health frameworks providing suggested approaches when conceptualizing and organizing local program, policy, and evaluation efforts. The MCAH program is responsible for selecting one activity per domain area. An Annual Report is submitted each fiscal year and provides details on how the SOW activities were addressed. MCAH completed the Title V Five Year Needs Assessment in June 2024. The results of the local data will be combined into the state results and reflected in the state mandated health directives outlined in the annual Scope of Work.

Through MCAH, the following services are provided to the community:

                     Safe to Sleep- Provide education to providers to increase knowledge and awareness of Safe Sleep practices. This is done through an annual Safe Sleep public service campaign annually in October.

                     Perinatal Community Coalition- Serves as Community Advisory Board, a program requirement. This group meets and shares information on a variety of subjects that pertain to the domains in the MCAH Scope of Work and identified in the Five Year Needs Assessment:

o                     Breastfeed Humboldt;

o                     Postpartum Support;

o                     Perinatal Substance Use;

o                     Vaccinations;

o                     Oral Health;

o                     Sexually Transmitted Infections;

o                     Managed Care/ Partnership Health Plan Updates;

o                     Other Home Visiting Services;

o                     Medi-Cal updates and changes to regulations;

o                     Other DHHS services and programs impacting the MCAH population.

                     Fetal Infant Mortality and Child Death- Conduct reviews with appropriate community partners.

                     Child Passenger Safety- Education for parents. Provide car seats and safety checks. Certify Car Seat Technicians.

                     SafeCare- Evidence-based, training program for parents of children ages 0-5 who are  at-risk for or have been reported for child neglect or physical abuse.

                     Perinatal Services Coordinator- Conducts outreach to local Obstetrical and Pediatric providers with referrals and linkage to services and resources.

                     Oral Health- Facilitates two community partner meetings, Dental Advisory Group and the Oral Health Leadership Team, to identify disparities and gaps in oral health services and work together to address the needs of our community. Provides care coordination services for families.

 

Title V is an automatic allocation. The Public Health Director would be attesting to the spending of Title XIX dollars. There are no legal terms to this agreement, and the grantee is a funding source.

FINANCIAL IMPACT:

Approval of the AFA with MCAH Division of the California Department of Public Health will allow DHHS-Public Health to be reimbursed up to an estimated maximum of $337,129 for services performed in accordance with the Scope of Work for fiscal year 2024-25.

 

This funding resides in Fund 1175, Budget Unit 420 - Maternal, Child, and Adolescent Health (MCAH). Funding in the amount of $337,129 was anticipated and included in the Fiscal Year 2024-25 proposed county budgets.

 

STAFFING IMPACT:

Approval of the AFA with the MCAH Division of the California Department of Public Health will maintain current staffing levels in the Humboldt Maternal, Child, and Adolescent Health (MCAH) program.

 

OTHER AGENCY INVOLVEMENT:

None

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

Your Board could choose not to approve the Agreement Funding Application. However, this is not recommended as it would limit the ability of Public Health to provide MCAH services in Humboldt County.
