RECOMMENDATION(S):

Recommendation

That the Board of Supervisors approve, and authorize the Chair of the Board to execute, the attached mental health organizational provider services agreement with Crestwood Behavioral Health, Inc. - Pathways regarding the provision of adult residential treatment services to eligible Medi-Cal beneficiaries residing in Humboldt County for fiscal years 2018-2019 through 2020-2021; and direct the Clerk of the Board to return two (2) fully executed original copies of the attached mental health organizational provider services agreement to the Department of Health and Human Services - Contract Unit.

DISCUSSION:

The Board of Supervisors has authorized the Department of Health and Human Services - Mental Health (DHHS - Mental Health) to operate the County of Humboldt’s Mental Health Plan.  The provision of specialty mental health services to all Medi-Cal beneficiaries residing in Humboldt County that meet necessary criteria is, and will remain, the responsibility of DHHS - Mental Health.  Therefore, since April 1998, DHHS - Mental Health has contracted with qualified specialty mental health providers, including Crestwood Behavioral Health, Inc. - Pathways (“Crestwood”), who have historically served Medi-Cal beneficiaries residing in Humboldt County.  DHHS - Mental Health has established a provider network with in-county as well as out-of-county providers for referrals of Medi-Cal beneficiaries residing in Humboldt County to ensure such beneficiaries have a choice of appropriate providers.

 

Crestwood operates a variety of facilities throughout California designed to serve diverse client needs.  Crestwood’s adult residential treatment facility located in Eureka, California, known as the Pathways program, serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health services to severely and persistently mentally ill adults.  The Pathways program is designed to support clients who are dually diagnosed with chemical addiction and psychiatric disorders in transitioning from Crestwood’s mental health rehabilitation center, a secure locked facility.

 

Clients placed Crestwood’s Pathways program are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee.  The SAFARI committee authorizes services, determines the most appropriate level of care and monitors client’s needs in the community upon discharge from the Pathways program.  When an initial placement at the Pathways program occurs, transition plans to a less restrictive placement are developed and reviewed monthly in order to transition the client to community livening as soon as may be clinically appropriate.  All mental health services provided through Crestwood’s Pathways program are delivered with a wellness focus, which includes, without limitation, concepts of recovery, resilience and independence. 

 

On February 23, 2016 (item C-11), the Board approved the current mental health organizational provider services agreement with Crestwood regarding the provision of adult residential treatment services through the Pathways program.  Crestwood has demonstrated expertise in providing assessment, individual therapy, family therapy, mental health plan development, mental health treatment services and medication support services.  Approval of the attached mental health organizational provider services agreement will allow eligible Medi-Cal beneficiaries residing in Humboldt County to receive adult residential treatment services from Crestwood through June 30, 2021.

 

The attached mental health organizational provider services agreement with Crestwood comes to the Board after the start date thereof due to inadvertent delays that occurred during the negotiation process.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached mental health organizational provider services agreement with Crestwood regarding the provision of adult residential treatment services to eligible Medi-Cal beneficiaries residing in Humboldt County through the Pathways program.

 

FINANCIAL IMPACT:

The attached mental health organizational provider services agreement with Crestwood has a maximum amount payable of One Million Six Hundred Twenty-One Thousand Five Hundred Dollars ($1,621,500.00) for the period of July 1, 2018 through June 30, 2021.  In no event shall the maximum amount paid under the attached mental health organizational provider services agreement with Crestwood exceed Five Hundred Forty Thousand Five Hundred Dollars ($540,500.00) per fiscal year for fiscal years 2018-2019 through 2020-2021.   However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce such maximum amount payable or terminate the attached mental health organizational provider services agreement in its entirety.

 

Expenditure appropriations related to the attached mental health organizational provider services agreement with Crestwood have been included in the proposed fiscal year 2018-2019 budget for DHHS - Mental Health Administration budget unit 1170-424.  Funding for the attached mental health organizational provider services agreement is made available through Medi-Cal financial participation and state realignment revenues.  As a result, approval of the attached mental health organizational provider services agreement with Crestwood will not impact the Humboldt County General Fund.

 

The recommended actions support the Board of Supervisors’ strategic framework by protecting vulnerable populations, supporting self-reliance of citizens and improving the safety and health of our communities.

 

 

OTHER AGENCY INVOLVEMENT:

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose not to approve the attached mental health organizational provider services agreement with Crestwood regarding the provision of adult residential treatment services to eligible Medi-Cal beneficiaries residing in Humboldt County.  However, this alternative is not recommended since it would require DHHS - Mental Health to directly provide the adult residential treatment services currently being provided by Crestwood, which is not feasible at this time.
