RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve the agreement with the County of Santa Cruz concerning MAA and TCM between the County of Humboldt and the County of Santa Cruz for the period beginning July 1, 2022, through June 30, 2025;

2.                     Authorize the Director of Public Health, or a designee thereof, to execute the agreement; and

3.                     Authorize the Director of Public Health or designee to execute any and all subsequent amendments and documents directly related to this agreement upon review and approval by County Counsel and Risk Management.

DISCUSSION:

The MAA/TCM Coordination and Claims Administration program provides administrative, programmatic, and fiscal oversight and support to MAA and TCM program participants on a countywide basis. The Department of Health and Human Services (DHHS) - Public Health serves as the Local Governmental Agency (LGA) for MAA and TCM claiming on behalf of Humboldt County.

 

The LGA draws down Federal Financial Participation revenues for DHHS to decrease local costs for eligible services and to assist in maintaining service levels. Services include case management, referrals, outreach, and program planning for Medi-Cal services.

 

Humboldt County has been participating in MAA and TCM since fiscal year (FY) 1997-98. Since this time the MAA and TCM programs have brought in funds to support community members. The MAA and TCM programs provide reimbursement for Medi-Cal outreach, client case management, and administrative costs associated with referring clients to Medi-Cal and Medi-Cal services.

 

This agreement is between the County of Humboldt and the County of Santa Cruz, the Host County for FY 2022-23 through FY 2024-25. The responsibilities of the Host County include collecting and disbursing funds for the MAA and TCM trust funds, paying Department of Health Care Services (DHCS) for the MAA/TCM administrative costs, paying the MAA/TCM LGA consultant costs, managing, and overseeing all procurement activities on behalf of the Consortium, and other duties and responsibilities as defined in the LGA Consortium Bylaws.

 

Currently, Public Health Nursing participates in the TCM program. Participating in the MAA program are: Public Health Nursing, Public Health Communicable Disease, Public Health Healthy Communities, and the Public Guardian. Approval of this agreement will allow all participating programs to continue receiving funds.

 

FINANCIAL IMPACT:

The agreement before your Board today will allow DHHS - Public Health to reimburse Santa Cruz County a percentage of the revenue DHHS - Public Health received in the prior fiscal year for services rendered. This agreement is fully funded by MAA and TCM state reimbursements. The maximum amount of the agreement is $71,500 over the term July 1, 2022 through June 30, 2025. Funding in the amount of $30,000 was budgeted in fiscal year 2022-2023 in Fund 1175, Budget unit 403, Medi-Cal Administrative Activities (MAA) and Targeted Case Management (TCM). The remainder of the participation fee will be included in FY 2023-24 and 2024-25 annual budgets. There is no impact to the County General Fund.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could decide not to approve the agreement with Santa Cruz County. However, DHHS-Public Health does not recommend this alternative as it would decrease services to a vulnerable population.
