RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached sub-grant agreement with Willow Glen Care Center regarding the establishment and administration of an Adult Crisis Residential Treatment Facility in Humboldt County for the period of July 1, 2022 through June 30, 2024;

2.                     Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached sub-grant agreement with Willow Glen Care Center, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) fully executed original copy of the attached sub-grant agreement with Willow Glen Care Center to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

As part of an ongoing effort to provide an adequate continuum of care for those individuals struggling with acute mental health crisis, the Humboldt County Department of Health and Human Services - Behavioral Health (“DHHS - Behavioral Health”), has been working to identify additional needed mental health treatment services within Humboldt County.  In early 2020 DHHS - Behavioral Health requested proposals from community providers for a Sub-Acute Transitional Mental Health Residential Treatment Facility.  On July 1, 2021, Willow Glen Care Center was awarded the Request for Proposal (RFP), which detailed the implementation of a Crisis Residential Treatment facility.  Since this time, DHHS - Behavioral Health has been working with Willow Glen Care Center to find a suitable building to provide this service.

 

As a result of the RFP submitted by Willow Glen Care Center, it was determined that a ten (10) Bed Crisis Residential Treatment Facility would provide a much needed resource in the Behavioral Health continuum of care.  Crisis Residential Treatment offers individuals stepping down from acute psychiatric placements, such as Sempervirens, or those at risk of needing acute psychiatric placement, a place to both reside and receive mental health treatment services such as Medication Support, Crisis Stabilization and Case Management.  The length of time an individual can stay at a Crisis Residential Treatment facility varies, but Crisis Residential Treatment can accommodate stays of thirty (30) to forty-five (45) days for individuals who need this level of stabilization.

As a result of these efforts, DHHS - Behavioral Health was awarded funding through the California State Assembly’s Upstream Investments for the North Coast Healthcare Systems Program in the amount of Two Million Dollars ($2,000,000.00) to help facilitate the “brick and mortar” and startup costs, needed for this Crisis Residential Treatment service.  The attached sub-grant Agreement will allow Willow Glen Care Center to receive a portion of this funding for expenses incurred in establishing an Adult Crisis Residential Treatment facility in Humboldt County.  

 

Willow Glen Care Center operates a variety of mental health treatment facilities, including Crisis Residential Treatment facilities, that are designed to serve diverse client needs, including, without limitation, several adult residential treatment facilities and secure mental health rehabilitation centers throughout Northern California.

 

DHHS - Behavioral Health currently contracts with Willow Glen Care Center for a continuum of long-term care options for individuals conserved by Humboldt County Public Guardian.  These facilities offer rehabilitative mental health services for severely and persistently mentally ill individuals.  All mental health services provided by Willow Glen Care Center focus on transitioning clients to independent living within their communities as soon as it is appropriate.

 

Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached sub-grant agreement with Willow Glen Care Center regarding the establishment and administration of an Adult Crisis Residential Treatment Facility in Humboldt County for the period of July 1, 2022 through June 30, 2024.  It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached sub-grant agreement with Willow Glen Care Center, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the establishment of an Adult Crisis Residential Treatment Facility in Humboldt County. 

 

The attached sub-grant agreement with Willow Glen Care Center comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract development process.

 

FINANCIAL IMPACT:

The attached sub-grant agreement with Willow Glen Care Center has a maximum amount payable of One Million Three Hundred Fifty-One Thousand Five Hundred Twenty-Five Dollars ($1,351,525.00) for the period of July 1, 2022 through June 30, 2024.  In no event shall the maximum amount paid under the attached sub-grant agreement with Willow Glen Care Center exceed One Million One Hundred Forty-Four Thousand Five Hundred Twelve Dollars ($1,144,512.00) for fiscal year 2022-2023 and Two Hundred Seven Thousand Thirteen Dollars ($207,013.00) for fiscal year 2023-2024.  However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached sub-grant agreement with Willow Glen Care Center in its entirety. 

 

Expenditure appropriations related to the attached sub-grant agreement with Willow Glen Care Center have been included in the approved fiscal year 2022-2023 budget for DHHS - Behavioral Health Administration budget unit 1170-424.  Funding for the attached sub-grant agreement with Willow Glen is available through California State Legislature allocation of funds to support the creation of a Crisis Residential Treatment Facility in Humboldt County received by DHHS - Behavioral Health.  As a result, approval of the attached professional services agreement with Willow Glen will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens, creating opportunities for improved health and safety and providing community-appropriate levels of service.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached sub-grant agreement with Willow Glen Care Center regarding the establishment of an Adult Crisis Residential Treatment Facility in Humboldt County.  However, this alternative is not recommended since it could require DHHS - Behavioral Health to leave clients placed within inpatient psychiatric facilities, or other higher level of care placements, or to be discharged to less clinically appropriate facilities or without needed services entirely.
