RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Helios Healthcare, LLC, doing business as Idylwood Care Center, regarding the provision of long-term residential skilled nursing and mental health treatment services to adults with chronic physical and mental illnesses for the period of July 1, 2022 through June 30, 2025;

2.                     Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Helios Healthcare, LLC, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return two (2) fully executed original copies of the attached professional services agreement with Helios Healthcare, LLC to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Behavioral Health (“DHHS - Behavioral Health”), in conjunction with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in locked and unlocked facilities.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate.

 

Helios Healthcare, LLC, doing business as Idylwood Care Center, operates the Courtyard at Idylwood, a locked skilled nursing facility, and the Gardens at Idylwood, an unlocked skilled nursing facility, located in Sunnyvale, California.  Helios Healthcare, LLC’s skilled nursing facilities provide medical and mental health treatment services to individuals over eighteen (18) years of age who require twenty-four (24) hour monitoring in a residential setting due to the severity of their physical and mental illnesses.  All of the medical and mental health treatment services provided at Helios Healthcare, LLC’s skilled nursing facilities are designed to transition clients to independent living within their communities, and are delivered with a wellness focus, which includes, without limitation, concepts of recovery and resilience.

 

Clients placed at Helios Healthcare, LLC’s skilled nursing facilities are monitored by the County’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (“SAFARI”) committee.  The SAFARI committee authorizes services, determines the appropriate level of care and monitors client needs in the community upon discharge.  When an initial placement at either of Helios Healthcare, LLC’s skilled nursing facilities occurs, transitional plans for less restrictive placement are put in place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate. 

 

The attached professional services agreement will allow eligible DHHS - Behavioral Health clients to receive skilled nursing and mental health treatment services at Helios Healthcare, LLC’s skilled nursing facilities for the period of July 1, 2022 through June 30, 2025.  It should be noted that the attached professional services agreement with Helios Healthcare, LLC comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract development process.

 

Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Helios Healthcare, LLC regarding the provision of long-term skilled nursing and mental health treatment services to adults with chronic physical and mental illnesses.  It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any future amendments to the attached professional services agreement with Helios Healthcare, LLC, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed skilled nursing and mental health treatment services.

 

FINANCIAL IMPACT:

The attached professional services agreement with Helios Healthcare, LLC has a maximum amount payable of Five Hundred Thirty-Two Thousand and Two Dollars ($532,002.00) for the period of July 1, 2022 through June 30, 2025.  In no event shall the maximum amount paid under the attached professional services agreement with Helios Healthcare, LLC exceed One Hundred Seventy-Seven Thousand Three Hundred Thirty-Four Dollars ($177,334.00) per fiscal year for fiscal years 2022-2023 through 2024-2025. However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Helios Healthcare, LLC in its entirety.

 

Expenditure appropriations related to attached professional services agreement with Helios Healthcare, LLC have been included in the approved fiscal year 2022-2023 budget for DHHS - Behavioral Health Adult Services budget unit 1170-496.  Funding for the attached professional services agreement with Helios Healthcare, LLC is available through state realignment and Mental Health Services Act revenues received by DHHS - Behavioral Health.  As a result, approval of the attached professional services agreement with Helios Healthcare, LLC will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens, creating opportunities for improved safety and health and providing community-appropriate levels of service.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board may choose not to approve the attached professional services agreement with Helios Healthcare, LLC regarding the provision of long-term skilled nursing and mental health treatment services to adults with chronic physical and mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
