RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Restpadd Inc. regarding the provision of adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of May 29, 2018 through June 30, 2019;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the professional services agreement with Restpadd Inc. regarding the provision of inpatient psychiatric services after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return a fully executed original copy of the attached first amendment to the professional services agreement with Restpadd Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Board of Supervisors has authorized the Department of Health and Human Services - Mental Health (DHHS - Mental Health) to operate the County of Humboldt’s Mental Health Plan. The provision of specialty mental health services to all Medi-Cal beneficiaries residing in Humboldt County that meet necessary criteria is, and will remain, the responsibility of DHHS - Mental Health. Therefore, since April 1998, DHHS - Mental Health has contracted with qualified specialty mental health providers, including Restpadd Inc., who have historically served Medi-Cal beneficiaries residing in Humboldt County. DHHS - Mental Health has established a provider network with in-county as well as out-of-county providers for referrals of Medi-Cal beneficiaries residing in Humboldt County to ensure that such beneficiaries have a choice of appropriate providers.

 

Psychiatric Health Facilities (PHF) offer an alternative to repeated psychiatric hospitalization. Adults placed at these facilities are stabilized, treated and transitioned to a variety of less restrictive placements as part of each individual’s long-term permanency plan.

 

Restpadd Inc.’s PHF, located in Redding, California, is an inpatient acute care treatment facility that provides multi-disciplinary evaluation, treatment, rapid stabilization and coordinated discharge planning services to adults from 18 to 64 years of age who are unable to maintain and safely function at home or in the community. When an initial placement at Restpadd’s PHF occurs, the client is stabilized and transitional plans are put into place and reviewed on a regular basis in order to transition the client to community living as soon as may be clinically appropriate. All psychiatric adult inpatient services provided at Restpadd Inc.’s PHF are delivered with a wellness and recovery focus.

 

On Nov. 19, 2018, the Board approved the current professional services agreement with Restpadd Inc. regarding the provision of adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of May 29, 2018 through June 30, 2019. The attached first amendment to the professional services agreement with Restpadd Inc. will increase the maximum amount payable thereunder in order to cover unexpected utilization of the adult inpatient psychiatric services provided by Restpadd Inc.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Restpadd Inc. regarding the provision of adult psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of May 29, 2018 through June 30, 2019. It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with Restpadd Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed inpatient psychiatric services.

 

FINANCIAL IMPACT:

The attached first amendment to the professional services agreement with Restpadd Inc. will increase the maximum amount payable thereunder from $55,000 to $77,000 for the period of May 29, 2018 through June 30, 2019. In no event shall the maximum amount paid under the professional services agreement with Restpadd Inc. exceed $21,120 for fiscal year 2017-18 and $55,880 for fiscal year 2018-19. However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Restpadd Inc. in its entirety.

 

Expenditure appropriations related to the attached first amendment to the professional services agreement with Restpadd Inc. have been included in the approved fiscal year 2018-19 budget for DHHS - Mental Health Administration budget unit 1170-424. Funding for this agreement is available through Medicaid Federal Financial Participation and Behavioral Health Realignment. As a result, approval of the attached first amendment to the professional services agreement with Restpadd Inc. will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, creating opportunities for improved safety and health and providing appropriate levels of services.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose to not approve the attached first amendment to the professional services agreement with Restpadd Inc. regarding the provision of adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County. However, this alternative is not recommended since it would require DHHS - Mental Health to directly provide the adult inpatient psychiatric services currently being provided by Restpadd Inc., which is not feasible at this time.
