RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve the attached Professional Services Agreement by and between the County of Humboldt and Open Door Community Health Centers for Fiscal Years 2025-26 through 2027-28; and

2.                     Authorize the Department of Health and Human Services - Public Health Director, or a designee there of, to execute the attached Professional Services Agreement by and between the County of Humboldt and Open Door Community Health Centers for Fiscal Years 2025-26 through 2027-28; and

3.                     Authorize the Department of Health and Human Services - Public Health Director, or a designee thereof, to execute any and all subsequent amendments, certifications, attestations, and documents directly associated with the attached Professional Services Agreement with Open Door Community Health Centers, after review and approval by County Counsel, Risk Management, and the County Administrative Office; and

4.                     Direct the Clerk of the Board to provide the Department of Health and Human Services - Contract Unit with one certified copy of the Board Order pertaining to this item.

DISCUSSION:

In 2019, the County of Humboldt Department of Health & Human Services (DHHS) - Public Health began contracting with the California Department of Public Health (CDPH) to implement public health activities related to monitoring, prevention, testing, and linkage to and retention in care activities for the most vulnerable and underserved individuals living with, or at high risk for hepatitis C virus (HCV) infection in collaboration with a non-profit, community-based organization (CBO). Allocated funds are to be used to develop and implement a public education and outreach program to raise HCV awareness in high-risk groups, including HCV counseling, education, and testing, as appropriate, into existing local state-funded programs including those addressing Human Immunodeficiency Virus (HIV), sexually transmitted infections (STIs), and other appropriate programs. Key strategic targets for HCV prevention and control are enhancement of surveillance and case follow-up for acute hepatitis C; testing, linkages to care, care coordination and assurance of treatment for HCV infection; partnerships to identify and address systemic barriers; and implementation of community-based services through partnerships between DHHS-Public Health and CBOs. This grant was renewed for Fiscal Years 2024-25 through 2027-28.

 

This grant requires that no less than 50 percent of the funds allocated be provided to a CBO that provides these activities and demonstrates expertise, history, and credibility working successfully in engaging the most vulnerable and underserved individuals living with, or at high risk for, HCV infection.

 

On Aug. 16, 2021, DHHS - Public Health posted a Request for Proposals (RFP) to seek a non-profit community-based organization capable of providing care and treatment to people living with HCV. The county received only one application; fortunately, it was from the county’s largest provider of these services, Open Door Community Health Clinics. The results of this partnership have enabled more than 450 people to receive HCV Care Coordination and Clinical Navigation services with nearly 150 of those completing treatment and final labs to confirm Sustained Viral Response (cured) in the first three years of the partnership.

 

This agreement will allow Public Health, Healthy Communities to continue providing services in collaboration with Open Door Community Health Centers throughout Humboldt County to minimize new HCV infections, and to maximize the number of people with HCV accessing appropriate care, treatment, and support.

Narrative Explanation of Financial Impact:

The agreement will be fully funded through the Hepatitis C Virus Collaboration Project, awarded by CDPH. The maximum amount of the agreement is $300,123 over the term July 1, 2025, through June 30, 2028. Funding in the amount of $100,041 will be included in the annual budget process for fiscal years 2025-26, 2026-27, and 2027-28 in Public Health Fund (1175), Hepatitis C Prevention Budget Unit (437006).

 

STAFFING IMPACT:

Approval of this agreement will allow DHHS - Public Health to receive matching funding to maintain the current level of staffing supported by the CDPH HCV Prevention and Collaboration grant.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

Your Board may choose not to approve the recommendations; however, it would result in a loss of both funding and services to the County.
