RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with A&A Health Services, LLC regarding the provision of long term residential mental health treatment services to adults with severe mental illness in an amount not to exceed One Million Ninety-Five Thousand Dollars ($1,095,000.00) for the period of July 1, 2025, through June 30, 2028; and
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with A&A Health Services, LLC after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Clerk of the Board to return one (1) fully executed original copy of the attached professional services agreement with A&A Health Services, LLC to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:
The Humboldt County Department of Health and Human Services - Behavioral Health (“DHHS - Behavioral Health”), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in locked and unlocked facilities. It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. DHHS - Behavioral Health is statutorily obligated to provide housing for individuals on Lanterman-Petris-Short (LPS) Conservatorship in a manner that is both protective of the conserved individual but also in the least restrictive setting. This mandate implies the need for multiple Long Term Care housing options to ensure individuals are appropriately placed and to have capacity to manage the number of conserved individuals.
A&A Health Services. LLC operates a two hundred twenty-five (225) bed adult residential treatment facility located in San Pablo, California. A&A Health Services, LLC’s adult residential treatment facility serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health treatment services for severely and persistently mentally ill individuals. All mental health treatment services provided at A&A Health Services, LLC’s adult residential treatment facility focus on transitioning clients to independent living within their communities as soon as may be appropriate.
Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with A&A Health Services, LLC regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2025, through June 30, 2028. It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with A&A Health Services, LLC after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed residential mental health treatment services.

The attached professional services agreement with A&A Health Services LLC has a maximum amount payable One Million Ninety-Five Thousand Dollars ($1,095,000.00) for the period of July 1, 2025, through June 30, 2028. In no event shall the maximum amount paid under this Agreement exceed Three Hundred Sixty-Five Thousand Dollars ($365,000.00) per fiscal year for fiscal years 2025-2026 through 2027-2028. However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with A&A Health Services, LLC in its entirety.
Expenditure appropriations related to the attached professional services agreement with A&A Health Services, LLC have been included in the proposed fiscal year 2025-2026 budget for DHHS - Behavioral Health Adult Services budget unit 1170-496. Funding for the attached professional services agreement with A&A Health Services, LLC is available through state realignment (61%) and Mental Health Services Act (39%) revenues received by DHHS - Behavioral Health.
STAFFING IMPACT:
Approval of the attached professional services agreement with A&A Health Services, LLC will not impact current staffing levels.
OTHER AGENCY INVOLVEMENT:
None
ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board may choose not to approve the attached professional services agreement with A&A Health Services, LLC regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses. However, this alternative is not recommended since it would require DHHS - Behavioral Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
