RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the attached Professional Services Agreement by and between the County of Humboldt and Conduent Healthy Communities Corporation for Fiscal Years 2025-2026 through 2027-2028 not to exceed $76,306.25 (Attachment 1); and
2. Authorize the Department of Health and Human Services - Public Health Branch Director, or a designee there of, to execute the attached Professional Services Agreement by and between the County of Humboldt and Conduent Healthy Communities Corporation for Fiscal Years 2025-26 through 2027-28; and
3. Authorize the Department of Health and Human Services - Public Health Branch Director, or a designee thereof, to execute any and all subsequent amendments, certifications, attestations, and documents directly associated with the attached Professional Services Agreement by and between the County of Humboldt and Conduent Healthy Communities Corporation for Fiscal Year 2025-26 through 2027-28 after review and approval by County Counsel, Risk Management, and the County Administrative Office.

DISCUSSION:
The County of Humboldt Department of Health and Human Services - Public Health Branch (DHHS - PHB), with the assistance of other Department of Health and Human Services (DHHS) branches, and other agencies in Humboldt County, publishes a Community Health Assessment (CHA) summarizing the health status of the county and also highlights disparities in health status of certain populations and their root causes.
DHHS - PHB identifies priority health concerns based on the data in the CHA and forms a Community Health Improvement Plan (CHIP) in order to address those health concerns. The branch used this information to create a web-based, public-facing Community Health Dashboard, which has allowed DHHS - PHB to share data from the CHA and CHIP with partner agencies and community members in efforts to track progress and align efforts in improving health outcomes in Humboldt County, through the Live Well Humboldt initiative. In addition, local partners have reported that the Community Health Data Dashboard has been a valuable source of information for grant applications and program development process. The dashboard has also been a vehicle for describing the work that various entities are doing to address and improve health priority areas identified in the 2022 CHIP: suicide prevention, substance use prevention and harm reduction, reducing homelessness and housing insecurity, and supporting healthy beginnings by working to prevent adverse childhood experiences and promote resiliency early in life.
DHHS - PHB is committed to providing access to trusted health data that is up to date and accompanied by analysis and context that helps Humboldt residents and others who visit the site understand the specific health problems facing the community and appreciate the collective work partners are doing to improve health status. In the past year DHHS - PHB have published stories about the important efforts local collaborative groups working in each of the CHIP priority areas.
On March 31, 2023, your Board signed a Professional Services contract with Conduent Healthy Communities Corporation for the creation of this data dashboard, as well as maintenance and hosting services. The agreement before your Board (Attachment 1)
Accordingly, DHHS - PHB recommends that the Board approve renewal of the contract with Conduent Healthy Communities Corporation and authorize the DHHS - PHB Director, or a designee thereof, to continue the Humboldt County Live Well Humboldt Data Dashboard with this vendor. This contract includes specifications for the development of an online e-CHA - a website-based Community Health Assessment format that utilizes the benefits of a multi-media format to expand the presence of Community Health Improvement in our community.

Narrative Explanation of Financial Impact:
The maximum amount payable by the County of Humboldt for any and all services provided, and costs and expenses incurred, during the term of the professional services agreement with Conduent Healthy Communities Corporation is $76,306.25. Maximum payment for Fiscal Year 2025-2026 is $41,523.00 and is approved and budgeted to Opioid Settlement Funding (1600-600).
The maximum amount payable for Fiscal Year 2026-27 is $17,391.63, and for Fiscal Year 2027-28 is $17,391.62, and is to be funded by the Partnership HealthPlan of California Grant (1175-400).
STAFFING IMPACT:
There is no additional staffing requested with this professional services agreement. The agreement does assist staff and community members in sharing the health of the community and providing analysis that assists grant and funding applications.
OTHER AGENCY INVOLVEMENT:
None.
ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board may choose not to authorize a renewal of services with Conduent Healthy Communities Corporation. However, this alternative is not recommended as it would prevent DHHS - PHB from meeting Public Health Accreditation requirements and prevent DHHS - PHB from efficiently sharing data to the community around health status of the community.
