RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached fourth amendment to the professional services agreement with Willow Glen Care Center regarding the provision of long-term residential mental health treatment services to adults with chronic mental illnesses for the period of July 1, 2016 through June 30, 2019; and

2.                     Direct the Clerk of the Board to return a fully executed original copy of the attached fourth amendment to the professional services agreement with Willow Glen Care Center to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (DHHS - Mental Health), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in a secure setting.

 

Willow Glen Care Center (Willow Glen) operates a variety of facilities throughout California designed to serve diverse client needs, including, an adult residential treatment facility and a secure mental health rehabilitation center, both of which are located in Yuba City, California.

 

Willow Glen’s adult residential treatment facility serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health services for severely and persistently mentally ill individuals. All mental health services provided by Willow Glen’s adult residential treatment facility focus on transitioning clients to independent living within their communities as soon as may be appropriate.

 

Willow Glen’s mental health rehabilitation center (MHRC) serves clients from Humboldt County and other areas who require 24-hour monitoring in a locked and secure environment due to the severity of their mental illness. DHHS - Mental Health clients placed at Willow Glen’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee. The SAFARI committee authorizes services, determines the appropriate level of care, and monitors clients’ needs in the community upon discharge. When an initial placement at Willow Glen’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate. All mental health treatment services provided at Willow Glen’s MHRC are delivered with a wellness focus that includes concepts of recovery and resilience.

 

On June 28, 2016 (item C-16), the Board approved the current professional services agreement with Willow Glen regarding the provision of long-term residential mental health treatment services at its adult residential treatment facility for the period of July 1, 2016 through June 30, 2019. The Board subsequently approved the first, second and third amendments to the professional services agreement with Willow Glen, which increased the maximum amount payable thereunder and adjusted the rates of compensation set forth therein, on August 15, 2017 (item C-12), June 12, 2018 (item C-11) and July 31, 2018 (item C-26). The attached fourth amendment to the professional services agreement with Willow Glen will allow for the provision of long-term residential mental health treatment services at Willow Glen’s MHRC for the period of January 1, 2019 through June 30, 2019.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached fourth amendment to the professional services agreement with Willow Glen regarding the provision of long-term residential mental health treatment services for the period of July 1, 2016 through June 30, 2019.

 

FINANCIAL IMPACT:

The attached fourth amendment to the professional services agreement with Willow Glen will increase the maximum amount payable thereunder from $647,752 to $660,000 for the period of July 1, 2016 through June 30, 2019.

 

In no event shall the maximum amount paid under the professional services agreement with Willow Glen exceed the following amounts:

                     $147,752 for fiscal year 2016-17

                     $250,000 for fiscal year 2017-18, and

                     $262,248 for fiscal Year 2018-19.

 

However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the professional services agreement with Willow Glen in its entirety.

 

Expenditure appropriations related to the attached fourth amendment to the professional services agreement with Willow Glen have been included in the approved fiscal year 2018-2019 budget for DHHS - Mental Health Adult Services budget unit 1170-496. Funding for the attached fourth amendment to the professional services agreement with Willow Glen is available through state realignment and Mental Health Services Act revenues received by DHHS - Mental Health. As a result, approval of the attached fourth amendment to the professional services agreement with Willow Glen will not impact the Humboldt County General Fund.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached fourth amendment to the professional services agreement with Willow Glen regarding the provision of long-term residential mental health treatment services to adults with chronic mental illnesses. However, this alternative is not recommended since it would require DHHS - Mental Health to place clients with chronic mental illnesses at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
