RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve and authorize the Chair of the Board to execute, the attached contract with Conduent Healthy Communities Corporation regarding the provision of web-based software and hosting services for the community-facing Department of Health and Human Services - Public Health Data Dashboard;

2.                     Through the Chair of the Board authorize the waiver in the language of the Nuclear-Free Humboldt County Ordinance Compliance - Conduent Healthy Communities Corporation requested the addition of “at the time of executing this agreement” in the clause to verify that they currently do not have any engagement with any nuclear activities; and

3.                     Authorize the Department of Health and Human Services - Public Health Director, or a designee thereof, to execute any and all subsequent amendments to the professional Services agreement with Conduent Healthy Communities Corporation, after review and approval by County Counsel, Risk Management, and the County Administrative Office.

DISCUSSION:

The Department of Health and Human Services-Public Health (DHHS-Public Health), with the assistance of other Department of Health and Human Services (DHHS) divisions and different agencies within Humboldt County, publishes a Community Health Assessment (CHA) summarizing the health status of the county and also highlights disparities in the health status of certain populations and their root causes. DHHS-Public Health identifies priority health concerns based on the data in the CHA and forms a Community Health Improvement Plan (CHIP) in order to address those health concerns.  A community-facing dashboard will allow DHHS-Public Health to share data from the CHA and CHIP with partner agencies and community members, aligning multiple efforts across agencies toward improving community health. This collective impact initiative is called “Live Well Humboldt.” In addition, DHHS intends to improve public access to data on programs that target community improvements on health and wellness that are impacted by social determinants of health (i.e., poverty, housing, etc.). The creation of this dashboard is also part of the requirements for the Public Health Accreditation.

 

DHHS-Public Health is committed to providing access to health data that will be developed, maintained and made publicly available via a community-facing dashboard. The dashboard will be hosted by an external vendor and will assist the County in sharing health assessment reports,  informing the community about health improvement strategies, and tracking progress.

 

On June 28, 2022, upon approval by your Board (Item D-28), DHHS-Public Health issued Request for Proposals (RFP) No. DHHS2022-04, which solicited proposals from qualified organizations to provide external services for the design and hosting of a web-based data dashboard for the CHIP.

 

On November 29, 2022, your Board approved the choice of this vendor after reviewing the five proposals that were submitted to DHHS-Public Health in response to RFP No. DHHS2022-04, as the vendor that most nearly met the project requirements and was within the proposed budget. The contract currently before the Board was negotiated with Conduent Healthy Communities Corporation.

Narrative Explanation of Financial Impact:

The total cost associated with entering into a professional services agreement with Conduent Healthy Communities Corporation regarding the provision/hosting of a web-based community-facing data dashboard the maximum amount is $79,825 for fiscal years (FY) 2022-23 through 2024-25.

 

Expenditure appropriations related to the proposed agreement with Conduent is made available through Epidemiology and Laboratory Capacity (ELC) - Enhancing Detection funding in Fund 1175, Budget Unit 455008 - ELC Enhancing Detection and California Equitable Recovery Initiative funding in Fund 1175, Budget Unit 400004-Health Equity. This expense was anticipated and included in the FY 2022-23 approved county budget and will be included in future FY county budgets. There is no impact to the County General Fund.

 

STAFFING IMPACT:

This contract will have no impact on staffing

 

STRATEGIC FRAMEWORK:

This action supports your Board’s Strategic Framework by creating opportunities for improved safety and health and providing community-appropriate levels of services.

 

OTHER AGENCY INVOLVEMENT:                     

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board may choose not to authorize the contract with Conduent Healthy Communities Corporation. However, this alternative is not recommended as it would prevent DHHS - Public Health from meeting Public Health Accreditation requirements and prevent DHHS - Public Health from efficiently sharing data with the community around the health status of the community.
