RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the attached Partnership HealthPlan of California Grant Agreement in the amount of $100,000 for fiscal years (FY) 2025-26 through 2027-28;
2. Authorize the Department of Health and Human Services (DHHS) - Public Health Branch Director, or a designee thereof, to execute the attached Partnership HealthPlan of California Grant Agreement for FYs 2025-26 through 2027-28; and
3. Authorize the DHHS - Public Health Branch Director, or a designee thereof, to execute any and all subsequent amendments, certifications, attestations and documents directly associated with the attached Partnership HealthPlan of California Grant Agreement after review and approval by County Counsel, Risk Management and the County Administrative Office.

DISCUSSION:
Partnership HealthPlan of California is a non-profit community-based healthcare organization that contracts with the State of California to provide Medi-Cal services to 24 counties in Northern California, under a County Organized Health System Model.
The California Department of Health Care Services (DHCS) has encouraged collaboration between Medi-Cal managed care plans and Local Health Jurisdictions (LHJs) to strengthen Community Health Assessments (CHAs) and Community Health Improvement Plans (CHIPs) as a means to improve population health management and align with the DHCS Quality Strategy.
Partnership HealthPlan of California has established a funding program to support counties in developing high-quality CHAs and CHIPs in alignment with state expectations and the requirements of the Future of Public Health Funding secured in the 2022 Budget Act (Chapter 249, Statutes of 2022), recognizing that many small and rural counties face barriers such as limited staffing, funding, or hospital infrastructure needed to complete this work.
A one-time grant in the amount of $100,000 has been awarded to Humboldt County to enable Live Well Humboldt to maximize outreach and secure a high level of participation by our community members in the CHA data-gathering and community feedback efforts. This grant will be used by DHHS - PHB to develop and track the CHA and CHIP in partnership with Live Well Humboldt.

Narrative Explanation of Financial Impact:
Approval of the Partnership HealthPlan of California Grant Agreement will allow DHHS - Public Health to accept a one-time, upfront payment of $100,000 for FY 2025-26 through FY 2027-28. It is anticipated that there is sufficient appropriation in the adopted budget for FY 2025-26 in the Public Health Administration budget unit (1175400) to support expenditures related to the Partnership HealthPlan of California Grant.

There is no staffing impact associated with this Grant Agreement.
OTHER AGENCY INVOLVEMENT:
There is no other agency involvement.
ALTERNATIVES TO STAFF RECOMMENDATIONS:
If we were not to accept this grant funding, the upcoming CHA would not have the rich community participation we hope to achieve, which is required to maintain our accreditation status. This would also affect the quality of the Community Health Assessment, on which we rely on developing the county-wide Community Health Improvement plan (CHIP).
