RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Restpadd, Inc. regarding the provision of adult psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of July 1, 2021 through June 30, 2023;
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Restpadd, Inc. after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Clerk of the Board to return one (1) fully executed original copy of the attached professional services agreement with Restpadd, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:
The Board of Supervisors has authorized the Department of Health and Human Services - Behavioral Health (“DHHS - Behavioral Health”) to operate the County of Humboldt’s Mental Health Plan. The provision of specialty mental health services to all Medi-Cal beneficiaries residing in Humboldt County that meet necessary criteria is, and will remain, the responsibility of DHHS - Behavioral Health. Therefore, since April 1998, DHHS - Behavioral Health has contracted with qualified specialty mental health providers, including Restpadd, Inc., who have historically served Medi-Cal beneficiaries residing in Humboldt County. DHHS - Behavioral Health has established a provider network with in-county as well as out-of-county providers for referrals of Medi-Cal beneficiaries residing in Humboldt County to ensure that such beneficiaries have a choice of appropriate providers.
Psychiatric health facilities offer an alternative to repeated psychiatric hospitalization. Adults placed at these facilities are stabilized, treated and transitioned to a variety of less restrictive placements as part of each individual’s long-term permanency plan. Restpadd, Inc’s psychiatric health facility, located in Redding, California, is an inpatient acute care treatment facility that provides multi-disciplinary evaluation, treatment, rapid stabilization and coordinated discharge planning services to adults between 18 and 64 years of age who are unable to maintain and safely function at home or in the community. When an initial placement at Restpadd, Inc’s psychiatric health facility occurs, the client is stabilized and transitional plans are put into place. These are reviewed on a regular basis in order to transition the client to community living as soon as may be clinically appropriate. All psychiatric inpatient services provided at Restpadd, Inc’s psychiatric health facility are delivered with a wellness focus, which includes, without limitation, concepts of recovery and resilience.
DHHS - Behavioral Health has previously contracted with Restpadd, Inc. on numerous occasions. On each occasion, Restpadd, Inc. demonstrated expertise in providing a full range of psychiatric inpatient services, including, without limitation, 24 hour nursing care, dietary services, medical, psychiatric, psychological and social assessments, crisis intervention, administration and supervision of psychotropic medications, individual and group therapy, art, recreational and vocational therapy, clinical laboratory services, social services and discharge planning services. Restpadd, Inc. is an out-of-county organizational provider that is only utilized when necessary to serve beneficiaries with the highest level of needs.
The attached professional services agreement will allow eligible Medi-Cal beneficiaries residing in Humboldt County to receive psychiatric inpatient services at Restpadd, Inc’s psychiatric health facility for the period of July 1, 2021 through June 30, 2023. It should be noted that the attached professional services agreement with Restpadd, Inc. comes to the Board after the start date due to inadvertent delays that occurred during the contract development process.
Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Restpadd, Inc. regarding the provision of adult psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of July 1, 2021 through June 30, 2023. It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with Restpadd, Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of needed psychiatric inpatient services.
FINANCIAL IMPACT:
The attached professional services agreement with Restpadd, Inc. has a maximum amount payable of $400,000.00 for the period of July 1, 2021 through June 30, 2023. In no event shall the maximum amount paid under the attached professional services agreement with Restpadd, Inc. exceed $200,000.00 per fiscal year for fiscal years 2021-2022 and 2022-2023. However, in the event local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Restpadd, Inc. in its entirety.
Expenditure appropriations related to the attached professional services agreement with Restpadd, Inc. have been included in the approved fiscal year 2021-2022 budget for DHHS - Behavioral Health Administration budget unit 1170-424. Funding for the attached professional Services agreement with Restpadd, Inc. is available through federal Medicaid financial participation and state realignment revenues received by DHHS - Behavioral Health. As a result, approval of the attached professional services agreement with Restpadd, Inc. will not impact the Humboldt County General Fund.
STRATEGIC FRAMEWORK:
The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, creating opportunities for improved safety and health and providing community-appropriate levels of services.

ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board may choose to not approve the attached professional services agreement with Restpadd, Inc. regarding the provision of adult psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County. However, this alternative is not recommended since it would require DHHS - Behavioral Health to directly provide the adult psychiatric inpatient services currently being provided by Restpadd, Inc., which is not feasible at this time.
