RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached fifth amendment to the professional services agreement with Crestwood Behavioral Health, Inc. regarding the provision of long-term residential mental health treatment services to adults with mental illnesses for the period of July 1, 2016 through June 30, 2019; and

2.                     Direct the Clerk of the Board to return a fully executed original copy of the attached fifth amendment to the professional services agreement with Crestwood Behavioral Health, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (DHHS - Mental Health), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill patients who require assistance in a secure setting.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. 

 

 Crestwood Behavioral Health, Inc. (Crestwood) operates a variety of facilities throughout California designed to serve diverse client needs, including, without limitation, an adult residential treatment facility and a secure mental health rehabilitation center, both of which are located in the City of Eureka.

 

Crestwood’s adult residential treatment facility, known as the Pathways program, serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health services for severely and persistently mentally ill persons.  All mental health treatment services provided by Crestwood’s Pathways program focus on transitioning clients to independent living within their communities.

 

Crestwood’s mental health rehabilitation center (MHRC) serves clients who require 24-hour monitoring in a locked and secure environment due to the severity of their mental illness. DHHS - Mental Health clients placed at Crestwood’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee. The SAFARI committee authorizes services, determines the appropriate level of care, and monitors clients’ needs in the community upon discharge. When an initial placement at Crestwood’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate. All mental health treatment services provided at Crestwood’s MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

On June 28, 2016 (item C-18), the Board approved the current professional services agreement with Crestwood for the period of July 1, 2016 through June 30, 2019. The Board subsequently approved four amendments to the professional services agreement with Crestwood, which increased the maximum amount payable thereunder and adjusted the rates of compensation set forth therein. The attached fifth amendment to the professional services agreement will increase the maximum amount payable thereunder in order to cover unexpected utilization of the residential mental health treatment services provided by Crestwood.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached fifth amendment to the professional services agreement with Crestwood regarding the provision of long-term mental health treatment services to adults with mental illnesses for the period of July 1, 2016 through June 30, 2019.

 

FINANCIAL IMPACT:

The attached fifth amendment to the professional services agreement with Crestwood will increase the maximum amount payable thereunder from $6,760,000 to $6,860,000 for the period of July 1, 2016 through June 30, 2019. In no event shall the maximum amount paid under the attached fifth amendment to the professional services agreement with Crestwood exceed $1,760,000 for fiscal year (FY) 2016-17, $2,500,000 for FY 2017-18 and $2,600,000 for FY 2018-19.  However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the professional services agreement with Crestwood in its entirety. 

 

Expenditure appropriations related to the attached fifth amendment to the professional services agreement with Crestwood have been included in the approved FY 2018-19 budget for DHHS - Mental Health Administration budget unit 1170-424. Funding for the professional services agreement with Crestwood is available through state realignment and Mental Health Services Act revenues received by DHHS - Mental Health. As a result, approval of the attached fifth amendment to the professional services agreement with Crestwood will not impact the Humboldt County General Fund.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached fifth amendment to the professional services agreement with Crestwood regarding the provision of long-term mental health treatment services to adults with mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
