Recommendation
That the Board of Supervisors:
1. Approve and execute the attached Professional Services Agreement between the Humboldt County Sheriff’s Office, the Humboldt County Probation Department, and Health Management Associates, Inc. for the planning, implementation, and operationalization of pre-release services, after review and approval of insurance documentation by Risk Management; and
2. Approve the attached supplemental budgets for fiscal year 2024/25, in Sheriff’s budget 1100243 and Probation’s budget 1100234 (4/5 Vote Required).

DISCUSSION:
The Humboldt County Probation Department (HCPD) and the Humboldt County Sheriff’s Office (HCSO) are jointly contracting with Health Management Associates, Inc. (HMA) to guide and facilitate Cal AIM implementation for both departments. CalAIM is a multi-year initiative by the California Department of Health Care Services (DHCS) to transform Medi-Cal, making it more equitable, coordinated, and person-centered to improve health outcomes and quality of life for Medi-Cal beneficiaries. Under the CalAIM Justice Involved initiative, both the Humboldt County Juvenile Hall and Humboldt County Correctional Facility will have mandates to provide pre-release services. There is funding tied to the implementation, however the funding requires an implementation plan and the deadline for that plan is June 2025. HMA would assist in the creation and implementation of that plan. This program is so new that there are few agencies/companies with the experience to consult with counties on CalAIM implementation.
The HCSO has a plan written and approved but lost the resource that was assisting with that project when she left employment with Wellpath. HCPD staff attended a webinar for another company offering this type of service however, they did not provide the full scope of services that HMA does and as of now, do not contract in Northern California. At this time, HMA appears to be the only contractor able to provide the full spectrum of services the county will require to push this project forward to completion as well as provide the necessary actuarial assessment. Due to these factors, County Purchasing has granted sole source approval for this contract.
This joint agreement spans fiscal years 2024-25, 2025-26 and 2026-27 with work anticipated to conclude in November, 2026. The contract is for $1,334,319 and is comprised of $690,485 for HCPD and $643,834 for HCSO. It is anticipated that the available PATH3 funding will cover the implementation costs so there is no anticipated impact to the general fund or any other county funding sources.
SOURCE OF FUNDING:
PATH JI round 3 grant will provide funding to support the planning and implementation of the provision of targeted pre-release Medi-Cal services to individuals in state prisons, county jails, and youth correctional facilities who meet the eligibility criteria as outlined in the CalAIM Section 1115 Demonstration approval. PATH funds will be available to support investments in personnel, capacity, and/or IT systems that are needed for collaborative planning and implementation in order to effectuate pre-release service processes.
The Humboldt County Probation Department was awarded $1,500,000, which is made available for use in stages, as follows:
• 10% ($150,000) within 45 days of award (received February 2025)
• 60% ($900,000) upon approval of implementation plan (due July 5, 2025)
• 15% ($225,000) upon submission of interim progress report (due January 15, 2026)
• 15% ($225,000) upon submission of final progress report (due no later than April 1, 2026)
The Humboldt County Sheriff’s Office was awarded $3,500,000, later adjusted to $1,396,000 which is made available for use in stages, as follows:
• 10% (350,000) within 45 days of award (July 2023) (10% of initial award)
• 60% ($627,200) upon approval of implementation plan (April 2025) (adjusted)
• 15% ($209,400) upon submission of interim progress report (adjusted)
• 15% ($209,400) upon submission of final progress report (adjusted)

Narrative Explanation of Financial Impact:
PATH JI round 3 grant will provide funding to support the planning and implementation of the provision of targeted pre-release Medi-Cal services to individuals in state prisons, county jails, and youth correctional facilities who meet the eligibility criteria as outlined in the CalAIM Section 1115 Demonstration approval. This funding will exceed the contract maximum and thus there is no impact to the county’s general fund or other revenue sources.

ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board may elect not to approve this agreement and supplemental budgets however this is not recommended. Both the Humboldt County Juvenile Hall and Humboldt County Correctional Facility will have mandates under this new program, and without the assistance of HMA it is unlikely that the HCPD or HCSO will be able to meet the criteria to access the full amount of available funding.
