RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Authorize the Department of Health and Human Services - Administration to submit a joint application with Aguilar Manor to the California Department of Social Services for Community Care Expansion program grant funding to support an increase in Residential Care Facilities for the Elderly capacity in Humboldt County;

2.                     Authorize the commitment of up to $150,000 in Department of Health and Human Services funding to the proposed Residential Care Facility for the Elderly expansion project;

3.                     Approve, and authorize the Chair of the Board to sign a letter of commitment required to apply for Community Care Expansion program grant funding, committing to provide up to $150,000 in Department of Health and Human Services funding; and

4.                     Direct the Clerk of the Board to provide the Department of Health and Human Services - Administration with one fully executed original copy of the attached Letter of Commitment for submission to the Community Care Expansion program.

DISCUSSION:

On July 27, 2021, Governor Newsom signed AB 133 (2021) which authorized the California Department of Health Care Services (DHCS) to establish the Behavioral Health Continuum Infrastructure Program (BHCIP) to award approximately $2.1 billion to construct, acquire, and expand properties and invest in mobile crisis infrastructure related to behavioral health. The California Department of Social Services (CDSS) oversees the companion component, Community Care Expansion (CCE), established under AB 172 (2021). CCE is focused on the acquisition, construction, and rehabilitation of adult and senior care facilities that serve Supplemental Security Income/State Supplementary Payment (SSI/SSP) and Cash Assistance Program for Immigrants recipients and other adults who are experiencing or at risk of homelessness.

 

Aguilar Manor, a Residential Care Facility for the Elderly (RCFE) located in Eureka, reached out to the Humboldt County Department of Health and Human Services (DHHS) with a proposal to expand their facility utilizing CCE program grant funding. DHHS has long partnered with Aguilar Manor to house DHHS clients in the Adult Protective Services and Behavioral Health programs. Currently, Aguilar Manor is licensed and approved for up to 15 residents, however they only have eight rooms in the facility. 

 

If awarded, CCE grant funding would allow Aguilar Manor to build a 2,100 sq/ft addition to the existing building that will include seven additional rooms, an office, storage space, two ADA bathrooms, a standard bathroom, and upgrades to staff quarters. Expansion funding would also be used to build three detached 990 sq/ft triplexes. Each unit will be a studio apartment with a full bathroom and kitchen. The total of nine units will allow for flexibility to house couples or individuals who require assistance to maintain self-sufficiency. Couples could live together when one spouse is providing care for the other, with the support services available when respite is needed. This expansion would help increase local housing capacity for some of the county’s most vulnerable residents.

 

The grant requires a 10% local match. Aguilar Manor intends to apply for $1.5 million. DHHS has identified $150,000 in Medicaid Intergovernmental Transfer (IGT) funding that can be used to satisfy the match requirement of the grant.

 

The grant application is due May 31, 2023.

Narrative Explanation of Financial Impact:

There are no direct costs associated with the initial submission of an application in response to the Request for Applications except for staff time. If the Community Care Expansion program grant is awarded, $150,000 from Budget Unit 1160-511-015 would be utilized to satisfy the 10% match requirement. Funding is available through Intergovernmental Transfer Funds (IGT) received by DHHS. The proposed budget for Fiscal Year 2023-24 includes appropriation that is available for this purpose.  As a result, if the grant is awarded no funding appropriation will be requested from Humboldt County General Fund.

 

STAFFING IMPACT:

 

Narrative Explanation of Staffing Impact:

No new positions are required. The expansion of housing and local respite care aligns with Behavioral Health, Housing and Adult programs within DHHS. Staff time associated with the partnership in application for funds will be covered by existing state, federal and local funds.

 

STRATEGIC FRAMEWORK:

This action supports your Board’s Strategic Framework priority of creating opportunities for improved safety and health, providing for and maintaining infrastructure, providing community appropriate levels of service, seeking outside funding sources to benefit Humboldt County needs and facilitating public/private partnerships to solve problems.

 

OTHER AGENCY INVOLVEMENT:                     

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to authorize the Chair of the Board to sign the attached Letter of Commitment or to authorize the commitment of up to one-hundred fifty-thousand dollars ($150,000) in DHHS funds to be used as match for the Community Care Expansion program grant. However, this alternative is not recommended, as it will prevent continued collaboration between DHHS and Aguilar Manor in pursuing critically needed infrastructure funding to build additional care beds for the elderly.
