RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to execute, the attached affiliation agreement with Donor Network West regarding the provision of organ, eye and tissue recovery and donation services at the County of Humboldt's psychiatric health facility;
2. Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to, and any other agreements, certifications, attestations and documents directly associated with, the attached affiliation agreement with Donor Network West, after review and approval by County Counsel, Risk Management and the County Administrative Office; and
3. Direct the Clerk of the Board to return one fully executed original copy of the attached affiliation agreement with Donor Network West to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:
The County of Humboldt's psychiatric health facility, Sempervirens, is a 16-bed psychiatric hospital providing around-the-clock inpatient care to individuals requiring psychiatric hospitalization. Sempervirens is operated by the Humboldt County Department of Health and Human Services - Behavioral Health (DHHS - Behavioral Health), and is licensed both as a psychiatric health facility by the California Department of Health Care Services and as an acute psychiatric hospital by the Centers for Medicare and Medicaid Services.
